This form documents discrimination and harassment based on protected class, sexual misconduct, and child abuse/neglect. If there is an immediate risk to health or safety, please dial 911 or contact crisis resources. Confidential, non-confidential, and 24-hour crisis resources can be found in the Resources tab.

IMPORTANT NOTE: The Title IX Office will review every report received to identify whether the conduct reported meets the definitions in the BSU Board of Governors Policy. Reports alleging inappropriate, unprofessional, or prohibited conduct that do not involve a potential violation of BSU B.O.G. policy may be referred to campus partners such as Human Resources, Student Conduct, Office of University Counsel, etc. It is important to include as much detail as possible to ensure your concerns are appropriately reviewed.

You can use this form to submit a report about your own experience or on behalf of an impacted party. To fulfill a duty to report you must include your name and all known details about the incident, including name(s), date, location, and description.

This is an incident report not a complaint form. This incident report may not, on its own, automatically initiate a Title IX investigation. A Title IX investigation will only begin after a complaint form is submitted, either by the complainant or by the Title IX Coordinator.
