To receive financial aid, federal law requires the student to be either a United States citizen/national or an eligible non-citizen.  Please complete the form by checking the appropriate box below, providing your signature, date and submitting the required documentation with this form to the Financial Aid Office, Bluefield State University, 219 Rock Street, Bluefield WV 24701.
Eligible Categories
Required Documentation: U.S. Birth Certificate or current U.S. Passport (except limited passports); or Certificate of naturalization (Form N-550 or N-570); or Certificate of Citizenship (Form N-560 or N-561); or Documentation of Birth of Citizen Abroad (Form FS-240, FS-545 or DS-1350)

 
Required Documentation: Permanent Resident Card (Form I-551), Resident Alien Card (Form i-1551); or Arrival or Departure Record (CBP Form I-94), Departure Record (Form I-94A) with proper endorsement of lawful admission for permanent residence.  Visa in an unexpired foreign passport with a U.S. Customs and Border protection (CBP) inspector admission stamp and the statement: “UPON ENDORSEMENTSERVES AS A TEMPORARY i-551 EVIDENCING PERMANENT RESIDENCE FOR 1 YEAR” or U.S. Travel Document (Form I327) with stamp “Permit to Re-enter”.

 
Required Documentation: Arrival or Departure Record (Form I-94 or Form I-94A) with stamp showing admission as refugee under Section 207 of the Immigration and Nationality Act (INA) with proper endorsement of lawful admission.

 
Required Documentation: Arrival or Departure Record (Form I-94A) with stamp showing admission as asylee under Section 208 or the Immigration and Nationality Act (INA)

 
Required Documentation: Arrival or Departure Record (Form I-94A) with stamp indicating “Alien paroled into the U. S. pursuant to Section 212 (d)(5)”.

 
Required Documentation: Victim of Human Trafficking must submit certification from the Department of Health and Human Services (HHS), spouse, child or parent of victim must submit a copy of the victim’s certification letter and a copy of their T-visa.

 
Required Documentation: Form I-94 stamped: “Cuban-Haitian Entrant (Status Pending).  Reviewable January 15, 1981.  Employment authorized until January 15, 1981.”

 
Required Documentation: Documentation of Canadian birth and student has Native American blood: “Band card” issued by Band Council of a Canadian Reserve or Department of Indian Affairs in Ottawa; or birth or baptismal record; or affidavit from tribal official or other individual knowledgeable about student’s family history; or identification from recognized Native American provincial or territorial organization
