West Virginia Governor Jim Justice has mandated COVID-19 testing for all students, faculty and staff at West Virginia colleges and universities, prior to the beginning of fall semester classes (BSC fall classes begin on Thursday, August 13, 2020).
Through the collaborative efforts of QLabs; Bluestone Health Association, Inc.; and the College, COVID-19 testing for BSC students and employees will be offered free-of-charge on Monday, August 10, 2020, beginning at 8 am in the Ned Shott Gym. Before starting fall semester face-to-face classes, BSC students and employees must provide evidence that they’ve tested negative for COVID-19, through the results of the on-campus testing on August 10 or through a COVID-19 negative test conducted on or after August 5, 2020.
To participate in the COVID-19 testing at Bluefield State on August 10, BSC students and employees must complete and provide at the test site: 1) a Health Insurance Portability And Accountability Act (HIPAA) Release form, 2) a Consent For Treatment form, and 3) a Coronavirus Exposure form (all of which can be accessed at the COVID-19 Resources and Return to Campus Plan, located on the Bluefield State College home page: https://bluefieldstate.edu/homepage/covid-19)
Please download, complete, print and bring the forms when you arrive at the testing site, Monday, August 10.
