Notification of Intent

The Notification of Intent Form is not an approval form or a firm commitment. It reflects your interest in applying for a specific grant. Prior to working on a grant application, you should discuss with your dean/director and complete this form. Items for discussion with your dean/director should include the purpose/intent of the grant, the funding agency, estimated amount of the award to be requested, deadlines, start/end dates, and amount of match, if applicable. Interest Survey

This is a short survey that will give the RASP office an idea of the interests of Bluefield State College faculty and staff members. The RASP office will use this information to post potential funding opportunities on the RASP website and alert faculty/staff to specific opportunities.

Animal Handler’s Health Questionnaire

As part of the in-processing procedure, personnel working with animals or in laboratories where animals are handled or used are required to complete an Animal Handler’s Health Questionnaire (found on the RASP website on the “Forms and Templates” page). This questionnaire is forwarded to the Occupational Health Physician who will use it in performing a risk assessment for the employee.
