Downloadable Application - Official college transcript (must be eligible to return to all prior colleges)
- Official high school transcript and ACT/SAT scores if fewer than 26 earned credit hours are being transferred (Refer to First-Time Freshman Requirements)
- Immunization Record of MMR (if born after January 1, 1957)
